New patients are always welcome. Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.
Office Hours
- Monday and Friday 10 AM - 6 PM
- Tuesday and Thursday 11 AM - 7 PM
- Every other Saturday 9AM - 5 PM
Insurance
Cosmetic procedures are not covered by insurance. Medical visits will be covered, and most insurance plans are accepted - please call the office for our updated list.
Financing Option
Integrative Dermatology participates in CareCredit for cosmetic procedures.
Billing & Payment
Payment is expected for all copays at the time of the visit. If you have either no insurance or you are covered by an insurance company with which we do not participate, all fees must be paid at the time of visit. We accept payment by cash, check, VISA, Mastercard and American Express.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
Patient Forms
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
